Veterans with 100% P&T status receive VA healthcare at Priority Group 1 — the highest level. This typically means comprehensive VA coverage with no copays for most covered services. For many 100% P&T veterans, VA healthcare provides thorough, high-quality care for service-connected and non-service-connected conditions.
This is a significant benefit. It's not nothing. But it has specific limits that matter at 65.
IMPORTANT
100% P&T status does not exempt you from Medicare enrollment timelines. It does not protect you from Medicare's late enrollment penalty. It does not extend your enrollment window. It does not create a Special Enrollment Period. Medicare's rules apply equally regardless of your disability rating.
This surprises many veterans. The natural assumption is that a 100% rating means the government will take care of the rest. On the VA side, that's largely true. But Medicare is a separate federal program with its own rules — and it does not carve out an exception for 100% P&T veterans.
If you miss your Medicare Initial Enrollment Period without qualifying active employer coverage, in most cases you face the standard Part B late enrollment penalty — typically 10% per 12-month period you were eligible but not enrolled, permanent.
A 100% P&T rating does not eliminate this penalty. A 100% P&T rating does not create a Special Enrollment Period. A 100% P&T rating does not make the penalty negotiable after the fact.
The Part B late enrollment penalty is typically permanent — it stays with you as long as you have Part B. For a veteran who lives 20+ more years, even a one-year delay can mean thousands of dollars in added premiums.
Even with excellent VA Priority Group 1 coverage, many 100% P&T veterans choose Medicare for specific reasons:
Access to civilian emergency rooms and hospitals — VA coverage may not be accepted at civilian ERs in the same way Medicare is.
Geographic flexibility — veterans who travel, relocate, or live in areas with limited VA access need civilian coverage.
Specialist access without referrals — Medicare allows you to see any participating specialist. VA may require referrals, prior authorization, or may not have the specialist available.
Spousal protection — a veteran's Medicare coverage doesn't cover their spouse. Having your own Medicare enrolled and in order is separate from spousal coverage decisions. See our Spouse of a Veteran guide.
The question isn't whether your VA coverage is good — it likely is. The question is: what does Medicare provide that VA doesn't? And what does missing the enrollment window cost you permanently?
For most 100% P&T veterans, the cost of Medicare Part B is significantly outweighed by the access, flexibility, and security it provides.
